Job summary

Powerback Rehabilitation seeks a Physical Therapist for full-time, in-home outpatient care in Glastonbury, CT. You will evaluate and treat older adults in the patient’s home or in living facilities, developing personalized therapy plans and coordinating with the rehab team.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy or equivalent.
• Bachelor’s degree in Physical Therapy plus 5 years of PT experience.

Responsibilities

• Provide outpatient rehabilitation services in the patient's home or in an independent living facility.
• Evaluate, plan and deliver physical therapy programs to facilitate rehabilitation.
• Educate patients and caregivers and coordinate referrals to other disciplines as needed.
• Supervise PT assistants and students where applicable and maintain documentation.
