Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Must have a Master's degree in Physical Therapy; or a Master's in a PT-related field plus Bachelor's in PT; or 5 years of PT experience with a Bachelor's.
• U.S. licensure requirements must be met to practice in home-based settings.

Responsabilités

• Provide outpatient rehabilitation services in in-home settings for older adults.
• Evaluate, plan, and deliver PT programs to restore function and independence.
• Educate patients and caregivers on home exercise programs and safety.
• Coordinate referrals to other disciplines as needed.
• Maintain accurate documentation and scheduling in compliance with licensure.
