Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home care. This part-time/per diem role supports clients to restore strength, balance, and independence in their own homes.

You will perform in-home evaluations, design personalized plans, educate families on exercises and safety, collaborate with a multidisciplinary team, and document care using a mobile EMR. MA license, DPT/MPT, CPR, and reliable vehicle required.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certification.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using a mobile EMR.
