Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist (PT) for part-time/per diem in-home visits. You will evaluate patients at home, develop personalized care plans, and deliver interventions to improve strength, balance, mobility, and safety.

CPR certification and a MA license are required. Enjoy a flexible schedule, mileage reimbursement, and access to mobile EMR, with support from a responsive clinical leadership team.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certification.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
