BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to join our in-home care team on a part-time/per diem basis. You will assess clients in their homes, develop personalized treatment plans, and provide interventions to improve strength, balance and mobility while ensuring safety and dignity.

A MA PT license and CPR certification are required, with 1+ year preferred. Flexible scheduling and mileage reimbursement are offered to support ongoing patient care.

Competenze

• Active MA PT license in good standing.
• DPT or MPT from an accredited program.
• CPR certification required.
• 1+ year of PT experience preferred (new grads welcome).

Mansioni

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile EMR.
