Job summary

Liberty Home Care is seeking an experienced Home Care Physical Therapist (PT) for full-time work serving Robeson and Scotland Counties in North Carolina. The role focuses on delivering direct treatment, evaluating new patients, and planning individualized plans of care in a home health setting.

The ideal candidate will hold a bachelor’s degree in physical therapy, be licensed in the state, and have ongoing CEU credits with travel readiness to cover assigned territories.

Qualifications

• Bachelor’s degree in physical therapy from an accredited program.
• Current licensure in the state where therapy is provided.
• Minimum twelve hours of continuing education annually.
• Willingness to travel to Robeson & Scotland Counties as required.

Responsibilities

• Provides patient care services along with direct treatment and follow-up.
• Evaluates new patients with physician orders and develops a plan of care.
• Documents patient care data accurately and timely to show progress.
• Submits daily/weekly visit schedules to the Scheduler.
• Communicates with intercompany contacts and care teams as needed.
• Coordinates with the Patient Care Coordinator on referrals and treatment plans.
• Provides in-service education and supervision as needed; supports students and staff.
