Liberty Home Care in Brunswick County, NC is seeking an experienced Home Care Physical Therapist (PT) for a full-time position. You will provide direct patient care at the home setting, evaluate new referrals, and develop individualized plans of care.

This role requires a Bachelor's degree in Physical Therapy, NC licensure, and at least 12 hours of CE annually; travel to patient homes is expected.

Formación

• Minimum bachelor’s degree in physical therapy from an accredited school and current licensure in the state where therapy will be provided.
• Prefer a minimum of one year’s therapeutic experience in a non-home health care environment.
• Strong knowledge of theory and concepts relating to physical therapy and ability to evaluate and treat patients with physician orders.
• Minimum of twelve hours of continuing education annually.
• Travel and working conditions as assigned.

Responsabilidades

• Provides patient care services along with direct treatment and follow-up.
• Evaluates patients new to service with orders and develops a plan of care.
• Documents patient care data accurately and timely, reflecting progress.
• Submits daily/weekly patient visit schedule to the Scheduler.
• Communicates with intercompany and intracompany contacts as needed.
• Discusses reimbursement policies with patient/family and treatment plans with care coordinator/physician.
