BAYADA Home Health Care in Scotland County is seeking an experienced Physical Therapist (PT) for a full-time role providing home health visits to adult and geriatric clients in Scotland and Robeson counties. One year of PT experience and NC PT license required.

You will work within a multidisciplinary team, perform assessments, develop care plans, educate clients and families, and document care using EMR.

Qualifications

• Current North Carolina PT license.
• Minimum 1 year PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently.
• Strong interpersonal and organizational skills.
• EMR experience preferred.

Responsabilités

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, balance, endurance).
• Develop and implement individualized care plans with exercises and gait training.
• Document observations and interventions in EMR.
• Educate clients and families on rehabilitative care and activities.
