Job summary

Liberty Home Care is seeking a dedicated Home Care Physical Therapist (PT) for full-time assignments in Nash & Halifax Counties. The role involves direct patient treatment, evaluation of new patients with physician orders, and development of individualized care plans to promote recovery at home.

The candidate must hold a Bachelor's degree in Physical Therapy and current licensure in the state, with strong documentation skills, CE requirements, and willingness to travel as needed.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited school.
• Current licensure in the state where therapy will be provided.
• At least 1 year of therapeutic experience preferred in a non-home health setting.
• Minimum of 12 hours of CE annually.
• Willingness to travel as assigned.

Responsibilities

• Provide patient care with direct treatment and follow-up.
• Evaluate new patients with physician orders and develop a plan of care.
• Document patient data accurately and timely to reflect progress.
• Submit daily/weekly visit schedules to Scheduler.
• Coordinate with care team on referrals and reimbursement policies.
• Educate staff and patients, and assist with orientation and training.
