Job summary

Liberty Home Care in Lumberton, NC seeks an experienced Home Care Physical Therapy Assistant (PTA) to provide patient-focused rehabilitation at home, under the supervision of a Physical Therapist. You will implement treatment plans, monitor progress, and coordinate with families to support recovery.

The role requires completing an accredited PTA program, current licensure, CPR certification, and the ability to travel across Robeson and Scotland Counties.

Qualifications

• Completion of an accredited physical therapy assistant program and current State licensure.
• Minimum of one year’s therapeutic experience in a non-home health care environment; two years preferred.
• Strong knowledge of theory and concepts relating to physical therapy and ability to treat patients with physician orders.
• CPR certified and provide strong communication and organization skills.
• Minimum of twelve hours of continuing education annually.
• Must accept travel and working conditions as assigned.

Responsibilities

• Provides patient care services along with direct treatment and follow-up as assigned by the Physical Therapist.
• Updates the care plan in coordination with the therapist and physician, and plans with the patient/family toward adjustment.
• Plans monthly patient re-evaluations with the therapist and coordinates with the therapist for scheduling.
• Documents patient care data accurately, reflecting progress or changes.
• Submits daily/weekly patient visit schedule to the Scheduler.
• Communicates with intercompany and intracompany contacts as needed.
• Discusses treatment progress with the therapist, patient/family, and physician as necessary.
• Coordinates referrals with Patient Care Coordinator for appropriate disciplines.
• Attends and participates in patient care conferences and mandatory in-services.
