Job summary

Qualifications

• Bachelor's degree in physical therapy and valid state licensure.
• Minimum one year of therapeutic experience in non-home health care.
• Continuing education of at least 12 hours annually.
• Willingness to travel as assigned.

Responsibilities

• Provides patient care services along with direct treatment and follow-up.
• Evaluates patients new to service with physician orders and develops a plan of care.
• Documents patient care data accurately and timely, showing progress.
• Submits the daily/weekly patient visit schedule to the Scheduler.
• Communicates with intercompany and intracompan y contacts.
• Discusses policies and treatment plans with patient/family and care team.
• Contributes to orientation and training of staff as needed.
