Qualifications

• Bachelor's degree in physical therapy and state licensure.
• Prefer 1 year of home health or equivalent experience.
• Strong understanding of PT theory and ability to evaluate patients with physician orders.
• At least twelve hours of continuing education annually.
• Willingness to travel as assigned.

Responsibilities

• Provide patient care services with direct treatment and follow-up.
• Evaluate new patients and develop care plans.
• Document patient care data accurately and timely.
• Submit visit schedules to Scheduler.
• Communicate with internal teams about policies and treatment plans.
• Coordinate referrals with Patient Care Coordinator.
• Provide in-service education and support students and staff as needed.
• Contribute to orientation and training of new employees.
• Conduct or assist with research as needed.
