Liberty Home Care is seeking a Home Health Physical Therapist (PT) for full-time work in Nash and Halifax Counties. You will provide direct patient care, evaluate new patients with physician orders, and develop individualized plans of care to support recovery at home.

The role requires a physical therapy degree, state licensure, and at least one year of experience outside home health, plus ongoing CEU hours to maintain proficiency and compliance.

Formación

• Bachelor's degree in physical therapy from an accredited school.
• Current licensure in the state where therapy will be provided.
• Minimum of one year's therapeutic experience outside home health care.
• Minimum of twelve hours of continuing education annually.

Responsabilidades

• Provides patient care services along with direct treatment and follow-up.
• Evaluates patients new to service and develops a plan of care.
• Documents patient care data accurately, thoroughly, and in a timely manner.
• Submits the daily/weekly patient visit schedule to the Scheduler.
• Communicates with intercompany and intracompany contacts.
• Discusses policies and guidelines with patient/family and care team.
• Coordinates referrals to appropriate disciplines.
• Provides in-service education and supports training as needed.
• Conducts research and assists with orientation of new employees.
