Job summary

Atrinity Home Health, part of LHC Group in the Optum family, seeks a Physical Therapist for Home Health. You will assess patient needs, develop treatment plans, and perform interventions to improve function and well-being in patients' homes.

Responsibilities include evaluating functional status, implementing the plan of care, ensuring compliance with state laws, coordinating with care teams, and documenting progress.

Qualifications

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of patient demographics.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists in developing the plan of care.
• Provides PT services within scope of practice and plan of care; coordinates with health care team.
