Job summary

Atrinity Home Health, part of LHC Group within the Optum family, is seeking a Physical Therapist Assistant to perform treatment and provide exercise instruction and patient education aimed at improving well-being under the established plan of care.

Responsibilities include following state PT laws, assisting with exercises and mobility, observing patient progress, coordinating with the care team, and educating patients and families.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.

Responsibilities

• Follow state laws governing provision of physical therapy in home care; follow treatment plan defined by supervising PT.
• Instructs and assists patients in active/passive exercise, gait training, ADL, and mobility.
• Observe, record, and report patient response; coordinate care with healthcare team.
• Instructs patient and family in therapy within scope of PTA.
• Function in diverse home situations regardless of patient demographics.
