Atrinity Home Health, a part of LHC Group and a member of the Optum family, is seeking a Physical Therapist in Home Health to assess patient needs, determine a treatment plan, and deliver interventions to improve mobility and overall well-being.

You will collaborate with physicians, nurses, and families to implement care plans, document progress, and ensure compliance with state guidelines while delivering compassionate, patient-centered care.

Qualificações

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsabilidades

• Evaluates the patient's functional status and physical therapy needs and assists the physician in care planning.
• Provides services within the scope of practice per state law and plan of care.
• Observes and reports patient response to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family about plan of care, limitations, and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes all evaluations and develops the PT plan of care per state guidelines.
• Reports outcomes and anticipated frequency of care.
• Ability to work in diverse home environments and with diverse patient populations.
