Job summary

Atrinity Home Health, part of the LHC Group within Optum family, is hiring a Physical Therapist for home health. You will assess functional status, mobility, endurance, and pain to help craft an individualized plan of care and implement interventions to improve patient well-being.

You will collaborate with the health care team, document progress, and ensure care aligns with state guidelines while benefiting from a comprehensive benefits package and performance recognition.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in developing the therapy plan of care.
• Provides PT services within the scope defined by state laws and plan of care.
• Observes and reports patient responses to treatment and changes in condition.
