Job summary

Ms HomeCare, a part of LHC Group, is seeking a Physical Therapist to provide in-home assessments and treatments for patients, focusing on mobility, endurance, pain, and overall well-being. The PT will develop and adjust plans of care in collaboration with the health care team.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance or access to dependable vehicle or public transportation.

Responsibilities

• Evaluates the patient’s functional status and PT needs.
• Assists physician in developing the therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses to treatment and condition changes.
• Completes all patient evaluations and documents progress notes.
• Instructs patient and family regarding plan of care and goals.
