Job summary

Atrinity Home Health, part of the LHC Group and Optum family, is seeking a Physical Therapist for home health. You will assess patient needs related to mobility, endurance, and pain; develop and implement a treatment plan; and evaluate progress in a home setting.

Responsibilities include evaluating function, coordinating with the care team, observing responses, and educating patients and families about the plan of care.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of client characteristics.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family regarding plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated care frequency.
