Job summary

Qualifications

• Must have a valid license in the state(s) of practice.
• Must have reliable transportation to patient residences.

Responsibilities

• Attends and contributes to patient care, staffing conferences and other required meetings under the direction of the Physical Therapist.
• Adheres to department and patient treatment schedules, and completes clinical and billing documentation as required.
• Collaborates with rehabilitation team to implement programs aligned with patient needs and capabilities.
• Acts as assistant to the Physical Therapist during tests, evaluations, and complex procedures.
• Instructs patients and caregivers in home-based PT techniques under supervision of the PT.
• Maintains timely documentation for all patients treated.
• Cleans, maintains, and reports needs for equipment repairs.
• Delivers patient‑centered service in a compassionate setting.
