Job summary

Powerback Rehabilitation To You in Toms River, NJ seeks a licensed Physical Therapist Assistant for in-home outpatient rehabilitation for older adults. The role emphasizes individualized care, collaboration with the rehab team, and flexible scheduling across patient homes and facilities.

The position requires a valid PT license, a PT degree (Master's preferred), and the ability to work in patients' homes with a strong commitment to functional recovery and patient education.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or Master's in a PT-related field with a Bachelor's in Physical Therapy.
• Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• Must have a valid driver's license or reliable transportation to patients' residences.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as required.
• Adhere to department and patient treatment schedules, complete timely clinical and billing documentation.
• Attend and contribute to patient care, staffing conferences and related meetings.
• Refer to other disciplines as needed to address patient needs.
• Collaborate with rehabilitation team to select appropriate therapeutic regimens.
• Adapt plan of care to individual patient needs.
• Promote safe environments and maintain equipment; order supplies as necessary.
• Provide home and community mobility assessments and recommendations.
