Job summary

Powerback Rehabilitation to You is seeking a licensed Physical Therapist to provide in-home outpatient rehabilitation for older adults. You will evaluate, design, and deliver individualized therapy programs in patients’ homes or living facilities, promoting independence and functional improvement.

The role requires a PT license, a master’s degree in PT (or PT-related with a bachelor’s), and reliable transportation for home visits.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy (or PT-related master’s with Bachelor’s).
• Driver’s license or reliable transportation to patient homes.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Develop and deliver individualized PT programs to restore, reinforce, and enhance function.
• Educate patients and caregivers, promoting independence and safe home mobility.
• Supervise PT assistants, students, and support personnel as permitted by state licensure.
• Maintain thorough documentation and scheduling in accordance with department standards.
