The role emphasizes collaboration with clinical teams, safety, and compassionate guidance, with travel to member homes as part of daily duties.

Qualifications

• Certified Peer Support Specialist or Certified Recovery Support Specialist required.
• Bilingual Spanish preferred.

Responsibilities

• Provides peer support and assists members in researching and accessing community resources.
• Assists in scheduling follow-up appointments and supporting wellness goals.
• Maintains safety and supports daily living activities (nutrition, budgeting, home tasks).
• Travels to members' homes for in-home services and may transport to appointments.
• Coordinates with clinical teams to align support with treatment plans.
