Easterseals Blake Foundation in Arizona is seeking a full-time Direct Support Professional to assist individuals with intellectual, developmental, and physical disabilities. You will provide personal care, community-based supports, and training in daily living and job skills.

The role emphasizes safety, compassion, and teamwork, with responsibilities that include transportation, medication assistance, and crisis support as needed. Commitment to inclusion and diversity is valued.

Qualifications

• High School Diploma or GED preferred.
• Three (3) months of caregiving experience preferred.
• Must be at least 21 years of age.
• Current, unrestricted driver’s license with limited violations.
• Ability to pass criminal background check and fingerprint clearance.
• Ability to obtain CPR, First Aid, and related certifications.

Responsibilities

• Assist individuals with personal care and hygiene needs.
• Perform housekeeping tasks and daily living activities.
• Provide transportation and community-based support for outings and appointments.
• Guide and support community participation, budgeting, exercise, and nutrition.
• Administer medications or assist with self-administration as prescribed.
