Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Must have a Master’s degree in Physical Therapy; or
• Must have a Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; or
• Must have a Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
• Must have a valid driver’s license or reliable, independent transportation to and from the patients’ residence.

Responsibilities

• The Physical Therapist will provide outpatient rehabilitation services in an in-home setting for older adults in Concord, NH and surrounding areas.
• Evaluate, develop, and deliver physical therapy programs to facilitate rehabilitation and independence.
• Supervise PTAs, temporary licensees, students and support personnel as required by state licensure.
• Coordinate with the rehabilitation team to select appropriate therapeutic regimens and document all services.
• Promote health and safety, educate patients and caregivers, and maintain timely documentation and billing.

Education

Master’s degree in Physical Therapy

Master’s degree in Physical Therapy related field + Bachelor’s in PT

Bachelor’s degree in Physical Therapy + 5 years PT experience
