Powerback Rehabilitation is seeking a Physical Therapist to provide outpatient rehabilitation services in an in-home setting for older adults in Southington CT and surrounding areas. The role includes evaluating and treating patients in their homes or in independent living facilities, developing customized therapy plans, coordinating with the rehab team, and teaching patients and caregivers.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master's degree in Physical Therapy is required, or equivalent combination.
• Driver's license or reliable transportation to patient homes.

Responsabilités

• Provide outpatient rehabilitation services in a home setting for older adults.
• Evaluate, develop, and deliver PT programs to facilitate rehabilitation.
• Coordinate care with other disciplines as needed.
• Instruct patients and caregivers in required skills for independence.
• Supervise PT assistants and support personnel as applicable.
• Maintain timely documentation and attend care conferences.
• Promote safety and order supplies/equipment when needed.

Formation

Master's degree in Physical Therapy

Bachelor's in Physical Therapy + Master's in related field

5 years PT experience or PT license equivalent
