Job summary

FOX Rehabilitation in Albuquerque, NM seeks licensed Occupational Therapists to deliver one-on-one care to older adults in private homes or senior living communities, improving mobility and independence through our Geriatric House Calls model.

The role offers autonomy to create and manage your own treatment schedule, with optional telehealth/virtual care to extend access and flexibility while collaborating with an interdisciplinary team for meaningful patient outcomes.

Qualifications

• Active Occupational Therapy license in the state of practice or eligibility to obtain it.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in homes or senior living communities.
• Perform evaluations, develop care plans, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and function.
• Document within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
