Job summary

FOX Rehabilitation is seeking licensed Occupational Therapists to deliver one-on-one care in Sun City, AZ, in private residences and senior living communities. You will assess, plan, and provide evidence-based OT services, with a focus on improving mobility and independence in older adults through the Geriatric House Calls model.

The role offers autonomy to manage your schedule, collaboration with an interdisciplinary team, and optional telehealth or virtual care to extend reach and continuity

Qualifications

• OT degree from an accredited Occupational Therapy program
• Current OT license in the state(s) of practice or eligibility to obtain
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one Occupational therapy to older adults wherever they call home (private residences, senior living communities, independent living, assisted living, memory care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Education
