Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to deliver one-on-one care in older adults’ homes and senior living communities. You will evaluate, plan, and administer evidence-based therapy while coordinating with a multidisciplinary team to maximize independence and quality of life.

Qualifications

• Licensed Occupational Therapist in state(s) of practice or eligibility to obtain license.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one occupational therapy to older adults in homes or senior living settings.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility, independence and safety.
• Document in FOX EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers and staff to promote aging in place.
