Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to provide one-on-one in-home and Senior Living Community care across Minnesota. You will conduct evaluations, design individualized plans, and deliver evidence-based therapy to improve mobility and independence for older adults.

In this role you’ll work autonomously with the flexibility to manage your schedule, while collaborating with a multi-disciplinary team.

Qualifications

• Valid Occupational Therapy license in state(s) of practice, or eligibility to obtain (OT licensure)
• Degree from an accredited Occupational Therapy program

Responsibilities

• Provide one-on-one occupational therapy to older adults in home or senior living settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence and functional daily living activities
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients and caregivers to promote safety and aging in place
• Offer telehealth/virtual care when appropriate to extend access
