Job summary

FOX Rehabilitation in Lakewood, CO is seeking licensed Occupational Therapists to deliver one-on-one care to older adults in private residences and senior living communities. You’ll enhance mobility, independence, and quality of life through our Geriatric House Calls model, with the flexibility to set your own schedule and work across home and community sites.

Join a clinician-led, collaborative team. Telehealth options may be incorporated to extend access.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one occupational therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support patient wellness, mobility, and independence.
• Document in FOX EMR per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
