Job summary

Adara Home Health is hiring an Occupational Therapist Case Manager to deliver skilled OT services in clients’ homes in Rochester, MN. You will plan, implement and document therapy under physician orders while coordinating with the clinical supervisor.

The role requires driving to client homes, strong communication, and the ability to work independently within office hours and travel within the coverage area.

Qualifications

• Licensed Occupational Therapist in Minnesota without restrictions.
• At least two years of healthcare experience; home care preferred.
• Strong interpersonal and teaching skills; capable of independent work.
• Current CPR recommended; access to reliable transportation.
• Willingness to travel to multiple client homes daily; valid driver’s license.

Responsibilities

• Perform initial and periodic client assessments to determine care needs.
• Provide OT treatments, evaluations and documentation under physician orders.
• Supervise COTAs, HHAs, LPNs and RNs as appropriate.
• Instruct families on home therapy programs and equipment use.
• Maintain confidential client records and communicate with the clinical team.
