Adara Home Health, Inc. is seeking an Occupational Therapist Case Manager to provide in-home therapy across Rogers and surrounding areas. You will assess, plan, and implement OT services per physician orders, educate families, and coordinate care with a Clinical Supervisor.

Qualifications

• Licensed Occupational Therapist in Minnesota with no license restrictions.
• Graduate of AOTA‑approved OT program and current AOTA certification.
• Minimum 2 years healthcare experience; home care preferred.
• National Board of Occupational Therapy certification recommended.
• Strong interpersonal, teaching, and communication skills.
• Ability to work independently with strong organizational skills.
• Access to a reliable vehicle or public transportation for daily visits.
• Valid driver's license and car insurance when using a personal vehicle.
• Background checks including MDH and OIG list compliance.
• CPR certification recommended.
• US Citizenship or valid Alien Work Permit.
• No disqualifying criminal history related to health services.

Responsibilities

• Perform OT assessments, plan, implement, and evaluate home-based therapy.
• Set treatment goals, update care plans, and reassess clients.
• Provide OT services and tests under physician orders.
• Translate exercises to ADLs and instruct use of equipment.
• Develop plans to modify equipment and home environments.
• Prepare progress notes and report to physician and Area Clinical Manager.
• Instruct and supervise COTAs, HHAs, LPNs, and RNs in therapy.
• Provide in-service education and participate in case conferences.
• Educate families to support home therapy progress.
• Maintain confidentiality and a safe home environment.
• Maintain productivity as per guidelines.
• Perform other duties as assigned.
