Job summary

Adara Home Health, Inc. is hiring an Occupational Therapy Case Manager to provide skilled OT services in clients' homes within Blaine, MN and Cambridge area. You will manage care plans, coordinate with physicians and adapt therapy to each client’s needs.

The role requires travel to client homes (1–3 hours per visit) during business hours and offers a base salary with productivity bonuses in a supportive team environment.

Qualifications

• Licensed Occupational Therapist in Minnesota.
• Graduate of AOTA approved program and current AOTA certification.
• At least two years of healthcare experience; home care preferred.

Responsibilities

• Perform assessments and implement OT plans of care.
• Set treatment goals and adjust care plans as needed.
• Provide OT treatments, evaluations and educational instruction in clients' homes.
• Instruct and supervise COTAs, HHAs, LPNs and RNs in therapy protocols.
