Job summary

Responsibilities

• Provide skilled OT services in patients' homes under OT supervision.
• Assist in evaluating and developing rehab plans under OT guidance.
• Prepare clinical and daily progress notes, submitted within 48 hours.
• Instruct patients and families in exercises and self-care activities.
• Identify potential adverse drug effects and report to the OT.
• Record patient progress and coordinate with OT and physician on outcomes.
• Attend rehab department meetings and assist with discharge summaries.
• Coordinate with supervisor to schedule home visits.
