Job summary

Clearwater is seeking an Occupational Therapy Assistant to provide skilled services in patients' homes under the supervision of an Occupational Therapist. You will assist with evaluation, care planning, and periodic re-evaluations, and help educate patients and families on exercises and self-care.

Responsibilities include preparing progress notes, collaborating with the OT and physician, and coordinating discharge summaries and follow-up visits as part of a multidisciplinary team in home health

Qualifications

• Graduated after completion of an OTA program accredited by ACOTE/AOTA or successor.
• NBCOT entry-level certification required or completed.

Responsibilities

• Provide skilled occupational therapy services in patients' homes under OT supervision.
• Assist in evaluation and development of the rehabilitative plan of care and periodic re-evaluation.
• Prepare clinical and daily progress notes and periodic summary reports.
• Instruct patients and families in therapy exercises and self-care.
• Document patient response and report changes to OT and physician.
• Discharge planning support and coordination of visits.
