Job Details

First Senior Home Health Tampa – Tampa, FL 33634. Position: Home Health Occupational Therapist Assistant (COTA). Full‑time and PRN positions available.

Duties
  • Provide skilled occupational therapy service, on an intermittent basis, to patients in their homes, under the supervision of the Occupational Therapist (OT).
Responsibilities
  • Under the direction of the OT, provide patient care services delegated by the OT.
  • Assist in evaluation, development, and periodic re‑evaluation of the rehabilitative plan of care as required.
  • Participate in the preparation of clinical and daily progress notes, submitted within 48 hours; also prepare periodic summary reports.
  • Instruct patient and family, assist in teaching exercise programs, and provide therapeutic self‑care and creative activities to improve independence and functioning.
  • Identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contraindicated medication and report to the OT.
  • Observe and record activities in the clinical record, report patient reaction to treatment and any changes in condition to the OT and physician.
  • Consult with OT regarding outcome of home visits and report physical findings in written form.
  • Assist in evaluation and development of the rehabilitative plan, and periodic re‑evaluation as indicated.
  • Instruct other health team personnel, including Home Health Aides, and family members, on phases of occupational therapy.
  • Attend rehabilitation department staff meetings and case conferences.
  • Instruct patient in use of wheelchairs, braces, splints, prosthetic and orthotic devices.
  • Participate in discharge activities and assist the OT in completing the Occupational Therapy Discharge Summary.
  • Confirm weekly scheduling of visits with Supervisor/Clinical Manager to coordinate agency visits.
  • Engage in self‑development through educational programs, workshops, conferences, active participation in professional organizations, and individual research.
  • Assume other duties deemed appropriate and necessary by the OT.
Qualifications
  • Graduated from a program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association (AOTA) or successor organizations.
  • Eligible to take or have successfully completed the entry‑level certification examination for Occupational Therapy Assistants administered by the National Board for Certification in Occupational Therapy (NBCOT).
  • Licensed or registered by the state of Florida.
  • Criminal background check completed.
  • Current CPR certification (online certification not accepted).
Job Conditions
  • Must be able to drive and access patients’ homes, which may not be wheelchair accessible.
  • Hearing, eyesight, and physical dexterity sufficient to perform physical assessment and patient care.
  • May be required to bend, stoop, reach, and move patient weight up to 250lb; lift/carry up to 30lb.
  • Effective communication in English required.
