Sr Companies seeks an Occupational Therapy Assistant to provide skilled OT services in patients' homes in the Tampa area under supervision of an OT. The position emphasizes evaluation support, patient education, and thorough documentation.

Requirements include graduation from an ACOTE-accredited OTA program, eligibility for NBCOT certification, Florida licensure, and current CPR certification. A background check is required, and driving to patient homes is expected.

Qualifications

• Graduated from ACOTE-accredited Occupational Therapy Assistant program.
• Eligible to take NBCOT certification exam.
• Must be licensed or registered by the state of Florida.
• Must have a criminal background check.
• Must have current CPR certification (online certification not accepted).

Responsabilités

• Under OT supervision, provide patient care services delegated by the Occupational Therapist.
• Assist in evaluation and development of the rehabilitative plan of care and periodic re-evaluation.
• Prepare clinical and daily progress notes; submit within 48 hours and provide periodic summary reports.
• Instruct patient and family in exercises and self-care activities to improve independence.
• Document patient progress and communicate findings to OT and physician.
• Instruct other health team members and family members on therapy portions as appropriate.
• Attend department meetings and case conferences.
• Assist in discharge planning and completion of discharge summary.
