Responsibilities

• Provide skilled occupational therapy service under OT supervision, in patients' homes.
• Under OT direction, deliver delegated patient care and assist in rehabilitation planning.
• Assist in evaluation and periodic re-evaluation of the rehabilitation plan of care.
• Prepare clinical and daily progress notes, submitted within 48 hours; prepare periodic summary reports.
• Instruct patients and families in exercises and self-care activities to improve independence.
• Document observations in clinical records and report changes to OT and physician.
• Participate in discharge activities and complete Occupational Therapy Discharge Summary.
