Job summary

CenterWell Home Health is seeking an experienced occupational therapist to help patients regain strength, independence, and confidence in their homes. You will assess daily living skills, design retraining plans, collaborate with physicians, and guide patients and families through rehabilitation and discharge planning.

Requirements include current OT licensure, CPR certification, and a valid driver's license with insurance.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patients' daily living/work-related skills and develop therapeutic retraining programs.
• Confer with physicians and care team to develop/revise the Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, range of motion and function.
• Review retraining programs to meet patient goals and rehabilitation guidelines.
• Design community reintegration activities and coordinate with the interdisciplinary team.
• Document care observations, interventions and outcomes accurately.
