Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to deliver patient-centered rehabilitation in home settings. You will assess functional status, design retraining programs, and coordinate with physicians and the care team to enhance independence and quality of life.

Responsibilities include interpreting tests, developing care plans, documenting outcomes, and leading education for patients and families. A valid driver's license and reliable transportation are required.

Qualifications

• Current and unrestricted OT licensure.
• CPR certification required.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Assess and screen patients' daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the physician and health team to develop the Plan of Care.
• Provide therapeutic treatment and instruction to improve/restore strength, coordination, ROM, and function.
• Review and expand retraining programs to meet patient goals within the rehabilitation program guidelines.
• Consult with vocational team members to develop vocationally oriented plans.
