Job summary

Ohio Living is seeking an Occupational Therapist to perform evaluations and treatments for patients in accordance with physician orders and company standards. You will participate in care planning, discharge planning, and collaborate with physicians, caregivers, and the inter disciplinary team to ensure high-quality care.

The role requires an Ohio OT license, at least 1 year of OT experience in hospital or home health settings, and experience with geriatric patients.

Qualifications

• Current unencumbered Ohio OT license.
• At least 1 year experience as OT in hospital, long-term care facility, or home health.
• Experience with geriatric and occupational therapy patients.

Responsibilities

• Perform occupational therapy evaluations and treatments as ordered by physicians.
• Initiate contact with physician for orders and treatment plan as needed.
• Conduct home visits to implement the Plan of Care and improve function and safety.
• Maintain accurate patient records per guidelines.
• Participate in discharge planning and coordination with care team.
