Qualifications

• Graduate of an ACOTE-accredited OT program.
• Current license to practice occupational therapy for the state assigned.
• Eligible to take or has completed NBCOT certification exam.
• Current CPR certification.
• 1+ years related OT experience or regional clinical leader approval for mentored/ residency pathway.

Responsibilities

• Evaluate and treat patients in their homes to promote independence, safety, and improved quality of life.
• Collaborate with patients, families and interdisciplinary team to develop individualized treatment plans.
• Follow physician orders and agency policies in plan of care development.
• Ensure safety and effectiveness of the OT service delivery process.
• Complete documentation timely and accurately.
• Supervise home health aide when RN is not active in the case.
