Job summary

Compassus is seeking an Occupational Therapist for a home-based role in partnership with Providence at Home. You will deliver inpatient-level therapy in patients’ homes, perform assessments, create treatment plans, and coordinate discharges with the care team.

The position requires OT licensure and BLS certification, with a preference for 1 year of adult home-care experience. You will manage a caseload, supervise aides, and document promptly per guidelines.

Qualifications

• Bachelor's degree in Occupational Therapy or Master's degree in Occupational Therapy.
• Current, unencumbered OT license.
• National Provider BLS certification.
• Preferred: 1 year of OT experience with adults in home care.

Responsibilities

• Deliver comprehensive therapy services including assessment, intervention, care planning, implementation, and discharge planning.
• Evaluate clients' physical, cognitive, psychological, and perceptual functions for safe home functioning.
• Develop individualized plans of care with patients and families.
