Job summary

Beyond Home Health Care in New Jersey seeks a licensed Occupational Therapist to provide in-home and community OT services to adults with developmental disabilities.

The OT will evaluate functional performance, develop person-centered treatment plans, and deliver interventions that maximize independence in ADLs, IADLs, sensory processing, fine motor skills, and community participation, per NJ DDD guidelines.

Qualifications

• Active New Jersey OT license required.
• Graduate of ACOTE-accredited OT program.
• NBCOT certification preferred; required for new grads.
• Minimum 1 year clinical experience preferred.
• Current CPR/BLS certification.
• Clean background check and fingerprinting per NJ DDD.
• Reliable transportation and NJ driver’s license.
• Ability to pass DDD background checks (CRoff).

Responsibilities

• Evaluate and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one OT in home and community settings.
• Document services per DDD standards including notes and outcomes.
• Collaborate with coordinators, families, and the team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce goals.
• Adhere to DDD policies and HIPAA confidentiality standards.
• Maintain billing records per DDD rates and authorizations.
• Complete required DDD trainings.
