Job summary

Beyond Home Health Care is seeking a licensed Occupational Therapist to provide in-home and community-based OT services to adults with developmental disabilities across New Jersey.

The OT will assess functional performance, create person-centered treatment plans, and deliver interventions to maximize independence in ADLs, IADLs, sensory processing, and fine motor skills per NJ DDD guidelines.

Qualifications

• Current New Jersey OT license issued by the NJ State Board.
• Graduate of an ACOTE-accredited occupational therapy program.
• NBCOT certification preferred; required for new graduates.
• Minimum of one year clinical experience preferred; new grads with fieldwork considered.
• Current CPR/BLS certification.
• Clean background check and fingerprinting per NJ DDD requirements.
• Reliable transportation and valid NJ driver’s license.
• Ability to pass DDD background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one OT services in home and community settings.
• Document services per DDD standards with session/progress notes and outcomes.
• Collaborate with coordinators, families, caregivers, and the team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Maintain billing records per DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Danielle’s/Stephen Komninos’ Law, etc.).
