Job summary

Beyond Home Health Care is seeking a licensed Occupational Therapist to provide individualized, in-home and community-based OT services to adults with developmental disabilities in New Jersey.

You will evaluate functional performance, create person-centered treatment plans, and deliver interventions to maximize independence in ADLs, IADLs, sensory processing, and fine motor skills in accordance with NJ DDD guidelines.

Qualifications

• Current New Jersey OT license.
• Graduate of an ACOTE-accredited OT program.
• NBCOT certification preferred; required for new graduates.
• Minimum 1 year clinical experience preferred; fieldwork OK for new grads.
• Current CPR/BLS certification.
• Clean background check and fingerprinting per NJ DDD requirements.
• Reliable transportation and valid NJ driver’s license.

Responsibilities

• Conduct comprehensive home- and community-based OT evaluations and develop individualized treatment plans.
• Deliver one-on-one therapy in home and community settings.
• Document services per DDD standards, including progress and outcome measures.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Adhere to DDD policies, Danielle’s Law, Stephen Komninos’ Law, and HIPAA standards.
• Maintain billing records in line with DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Abuse/Neglect/Exploitation, etc.).
