Job summary

Beyond Home Health Care is seeking a licensed Occupational Therapist to provide in-home and community-based OT services to adults with developmental disabilities in New Jersey. You will evaluate functional performance, develop person-centered plans, and deliver interventions to maximize independence in ADLs, IADLs, sensory processing, and fine motor skills.

The role requires NJ OT licensure, ACOTE accreditation, and CPR/BLS. Per diem/contract with flexible scheduling and mileage considerations.

Qualifications

• Current New Jersey OT license is required.
• ACOTE-accredited occupational therapy program completed.
• NBCOT certification preferred; required for new graduates.
• CPR/BLS certification is required.
• Background check and fingerprinting per NJ DDD requirements.
• Reliable transportation and valid NJ driver's license.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP goals.
• Deliver one-on-one OT services in clients' homes and community settings.
• Document services per DDD standards: session and progress notes, outcomes.
• Collaborate with support coordinators, families, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Adhere to DDD policies, Danielle's/S KOMninos laws, HIPAA, and confidentiality.
• Maintain billing records consistent with DDD fee-for-service rates and authorizations.
• Complete required DDD trainings (Pre-Service, Abuse/Neglect/Exploitation, etc.).
