Job summary
Ignite Rehab (DBA Metro Physical & Aquatic Therapy) is seeking a motivated in-home Occupational Therapist to join our growing team. This per diem role serves Cliffside Park, NJ and surrounding towns, offering flexible scheduling and a supportive environment with opportunities for professional growth.
The ideal candidate is a licensed OT in New Jersey with strong communication skills and the ability to travel to patient homes.
Qualifications
- OT program graduate and NJ license or eligibility.
- Home health experience preferred; mentor provided for documentation and compliance.
- Ability to travel to patients' homes within the territory.
- Strong communication with patients, staff and management.
Responsibilities
- Accept and treat patients within your designated geographic area.
- Provide 1:1 treatment.
- Evaluate patients and develop individualized treatment plans.
- Deliver care aligned with standards, policies, and plan of care.
- Educate family and other caregivers on therapy techniques.
- Plan and carry out OT programs to maintain or restore function.
- Perform and document an initial exam and determine diagnosis prior to intervention.
- Provide timely documentation for each visit.
- Evaluate effects of treatment and adjust accordingly.
- Willingness to treat geriatric patients.
