Job summary

Ignite Rehab (DBA Metro Physical & Aquatic Therapy) is seeking a motivated in-home Occupational Therapist to join our growing team. This per diem role serves Cliffside Park, NJ and surrounding towns, offering flexible scheduling and a supportive environment with opportunities for professional growth.

The ideal candidate is a licensed OT in New Jersey with strong communication skills and the ability to travel to patient homes.

Qualifications

• OT program graduate and NJ license or eligibility.
• Home health experience preferred; mentor provided for documentation and compliance.
• Ability to travel to patients' homes within the territory.
• Strong communication with patients, staff and management.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans.
• Deliver care aligned with standards, policies, and plan of care.
• Educate family and other caregivers on therapy techniques.
• Plan and carry out OT programs to maintain or restore function.
• Perform and document an initial exam and determine diagnosis prior to intervention.
• Provide timely documentation for each visit.
• Evaluate effects of treatment and adjust accordingly.
• Willingness to treat geriatric patients.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs

• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care

• Educate family members, home health aides, and other caregivers on proper therapy techniques

• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.

• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.

• Provide timely and accurate documentation for each visit

• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit

• Must be willing to treat geriatric patients.

Qualifications

• Occupational Therapist Program Graduate

• Licensed Occupational Therapist in the state of New Jersey or eligible.

• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.

• Ability to travel to patients’ homes within the assigned geographic territory

• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.
